Physician Referral Form

Do you have a patient with uncomplicated, musculoskeletal pain who could use an adjustment?  You can print this easy form for your referral.  My Chiro is an affordable, walk-in clinic, so your scheduler does not need to arrange an appointment.  We see patients on a p.r.n. basis, and we will contact you if physical examination reveals that manipulation is contraindicated, further medical examination is warranted, or if your patient’s condition does not improve as quickly as anticipated.
